An 84-year-old woman who suffered from long-standing non-insulin-dependent diabetes mellitus and hypertension was admitted to hospital for the management of severe lower abdominal pain and acute retention of urine. On physical examination tenderness was noted over the right lower quadrant and in the suprapubic area of the abdomen. She was found to have marked leukocytosis (31.5 × 109/L), hyperglycemia (18.5 mmol/L) and an increased serum creatinine level (230 μmol/L. An abdominal radiograph (Fig. 1) was obtained.
