    • Question 5: 1 = extremely; 2 = quite a bit; 3 = moderately; 4 = slightly; 5 = not at all

  • 9 patients refused the survey; 1 had died of cancer.

  • 2 patients refused the survey, and 1 was lost to follow-up.
